Chimney Fire/ M.A. Gallipolis FD
On 1-14-2012 at approx. 7:36 pm, D-4 responded to a mutual aid request from the Gallipolis FD to 127 Dewitt Dr. in Green Twsp. on a report of a chimney fire. Gallipolis FD was already on a working structure fire at the time of this call. Upon arrival units found that the fire had already extinguished itself. Units check the residence with a thermal imaging camera for possible fire extension, but determined all the fire had been contained to the chimney. There was no damage to the residence or any injuries. 4 trucks and 10 men resonded to the call with 3 men from the Gallipolis FD. All units cleared the scene and were back in the house by 2032.
MVC / Raccoon Twsp.
On 12-24-2011 at approx. 10:26 pm, D-4 responded to the area of US 35 and mile post 3 on a report of a one car MVC with injuries. Upon searching the area we were unable to locate the accident scene. After checking several other possible areas the Ohio State Highway Patrol located the accident in JAckson county near the truck stop.All units cleared and were back in the house by 22:48 pm.
Alarm Activation / Perry Twsp.
On 12-21-2011 at approx. 1:12 am, D-4 responded to 4352 Cora Mill Rd on an automatic alarm activation. Upon arrival units were unable to locate a cause for the activation. All units cleared the scene and were back in the house by 1:45 am.
CO Alarm / Raccoon Twsp.
On 12-20-2011 at approx. 6:26 pm, D-4 responded to 530 Trails End Rd on a report of a Carbon Monoxide alarm activation. Upon arrival units tested the residence with a meter and could not find any traces of carbon monoxide. All units cleared the scene and were back in the house by 7:14 pm.
Structure Fire / M.A. Gallipolis FD

On 12-17-2011 at approx. 3:20 am D-4 responded mutual aid to Gallipolis FD at 382 Maple Dr on a report of a structure fire. Units assited D-6 with water and manpower. The home owner was transported to Holzer of Gallipolis with smoke and thermal injuries by Gallia County EMS. D-4 units were back in the house by 5:35 am. Gallipolis units remained on scene for several more hours for mop up. Gallipolis FD has called in the Ohio Fire Marshals Office to investigate.
Alarm Activation / Village
On 12-16-2011 at approx. 9:46 am, D-4 responded to the Rio Grande Child Developement Center on an automatic alarm activation. Upon arrival units found that one of the children had activated a pull station. System was restored and all units were back in the house by 10:08 am.
Chimney Fire / Perry Twsp.
On 12-10-2011 at approx. 12:55 pm, D-4 responded to the area of 2389 Patriot Rd on a report of a chimney fire. Units were cancelled enroute by the original caller advising that they had extinguished the fire. All units were back in the house by 1:00 pm.
Alarm Activation / Village
On 12-9-2011 at approx. 2:08 am., D-4 responded to Boyd Hall at the University of Rio Grande on an automatic alarm activation. Upon arrival units found a pull station had been activated on the first floor. Alarm was determined to be malicious in nature and the scene was turned over to Campus Police. All units cleared the scene and were back in the house by 2:26 am.
MVC / Raccoon Twsp.
On 12-4-2011 at approx. 08:15 am, D-4 responded to the area of SR 325 North and Bandy Rd on a report of a one car MVC on its side. Upon arrival units found a pickup that had went off the roadway and coming to rest on it's side in a ditch. The operator of the vehicle was out upon arrival and declined having any injuries. Units assisted in traffic control until OSP arrived. All units cleared the scene and were back in the house by 08:41 am.

On 12-1-2011 at approx. 7:26 am, D-4 responded to the area of SR 588 and Bob Evans Farms on a report of a one car MVC vs. Deer. Upon arrival units found there to be no injuries. Units assisted in traffic control until OSP arrived. Units cleared the scene and were back in the house by 8:13 am.
Alarm Activation / Village
On 11-30-2011 at approx. 2:25 am D-4 responded to Boyd Hall at the University of Rio Grande on a report of an automatic alarm activation. Upon arrival Units found a pull station had been activated on the first floor of the building. The building was checked and the alarm was ruled malicious in nature. The scene was turned over to Campus Police. All units cleared and were back in the house by 2:31 am.
Alarm Activation / Village
On 11-29-2011 at approx. 7:58 pm D-4 responded to the University of Rio Grande Cafeteria on an automatic alarm activation. Upon arrival units found a pull station had been activated by unknown person. Units checked the building and found there to be no emergency. The scene was turned over to campus police. All units cleared the scene and were back in the house by 8:17 pm.
Car Fire / Village
On 11-29-2011 at approx. 10:28 am D-4 responded to the Bob Evans Farms Hall parking lot on a report of a vehicle on fire. Upon arrival units found a working fire in the engine compartment of the vehicle. Units extinguished the fire and there was no other vehicles damaged in the lot. All units cleared the scene and were back in the house by 11:00 am.
False Alarm/ Perry Twsp.
On 11-26-2011 at approx. 6:18 pm D-4 responded to the area of 1258 Brushy Point Rd. on a report of an unknown type of fire. Upon arrival units found the fire to be a controlled burn. All units cleared the scene and were back in the house by 6:42 pm.
Fire Alarm/ Perry Twsp.
On 11-23-2011 at Approx. 3:15 am D-4 respondedd to 4352 Cora Mill Rd. on a residential fire alarm activation. Upon arrival units found a smoke detector in the basement had malfunctioned. All units cleared the scene and were back in the house by 3:54 am.
MVC/ Perry Twsp.
On 11-19-2011 at approx. 8:50 pm. D-4 responded to the area of SR 141 and SR 325 on a report of a two car MVC with injuries. Upon arrival units found one vehicle just West of SR 325 on SR 141 and the other vehicle had fled the scene and crashed again just East of SR 325 on SR 141. Units assisted EMS in patient care and provided traffic control for OSP. The Gallia County EMS transported four patients to Holzer of Gallipolis with minor injuries. The scene was turned over to the OSP and all units were back in the house by 10:45 pm.
MVC / Raccoon Twsp.

On 11-16-2011 at approx. 6:52 pm D-4 responded to the area of 3809 SR 325 N. on a report of a truck vs. tree. Upon arrival units found a Dodge pickup truck had hit a tree head on and the occupant of the vehicle had fled the scene. Units assisted with traffic and scene safety then turned the scene ove to the Gallia county Sheriff's Dept. The Sheriff's Dept. later located the driver at an unknown location with no apperant injuries. All units were back in the house by 7:39 pm.
Brush Fire/ Raccoon
On 11-14-2011 at approx. 3:27 pm D-4 responded to the area of 1259 Holcomb Hollow Rd on a report of a brush fire. Upon arrival units found a 20' X 15' area of brush on fire. Fire was extinguished and all units were back in the house by 3:57 pm.
Structure Fire/ Raccoon Twsp.
On 11-12-2011 at approx. 5:44 am D-4 responded to the area of Shelton Rd. and Holcomb Hollow Rd. on a report of an old grain bin on fire. Upon arrival units found a 20 X 20 building already burnt to the ground. Units extinguished the remaining fire and secured the scene. The cause of the fire is still under investigation. All units were back in the house by 7:00 am.
Rescue/ Raccoon Twsp.
On 11-11-11 at approx. 9:55 am D-4 responded to the area of 1354 Adamsville Rd. to assist Gallia County EMS on a report of a male patient that had fallen from a tree stand. Upon arrival unit used ATV's to transport the patient out of the woods to the EMS unit. Patient was transported to Holzer of Gallipolis. All units were back in the house by 10:43 am.
MVC / Raccoon Twsp.
On 11-8-2011 at approx. 9:37 am D-4 responded just East of Adamsville Rd. on SR 588 on a report of a car vs. a truck. Upon arrival units found a car had T- boned a tanker truck while it was making a left hand turn. The car was wedged up underneath the truck with air bag deployment. All occupants were able to get out on their own and were treated by first responders on scene. Three patients were transported by Gallia County EMS and Life Ambulance to Holzer of Gallipolis. Units assited the Ohio State Highway patrol with traffic and were back in the house by 11:05 am.
Vehicle Fires/ Mutual Aid Madison Jefferson FD

On 11-2-2011 at approx. 12:29 am D-4 responded mutual aid with the Madison Jefferson FD to 2452 Centerpoint Rd on a report of a truck fire endangering another vehicle. D-4 was first on scene and found a fully involved Dodge truck on fire with a second vehicle partially involved. As unit were deploying hose from the apparatus a loud explosion occured from the truck that was on fire. The explosion realeased a pieace of metal debri that traveled approx. 30 - 35 feet through the air stricking Lt. Bob Shaw in the right leg area while he was standing at the pump panel. Lt. Shaw was excorted to the rear of the apparatus while other members from D-4 and MJFD extinguished the blaze. Lt. Shaw was rendered first aid by other members of the department until the Jackson County EMS arrived. Lt. Shaw received an extensive laceration to the ankle area and was transported to Holzer of Gallipolis by EMS, where he was treated and released. After an investigation of the incident it was determined that the metal object that hit Lt. Shaw was a large piece of the vehicles drive shaft that had exploded. MJFD members continue to investigate the cause of the fire itself. All units were back inthe house by 1:52 am. ((( more pictures can be scene in the photo albums )))
Structure Fire/ Mutual Aid D-12
On 10-31-2011 at approx. 1:44 pm D-4 responded mutual aid with D-12 to 2949 Mt. Olive Rd in Springfield Twsp. on a report of a fully invovled structure fire. Upon arrival units assisted D-12 with water support and manpower. D-4 used approx. 5000 gallons of water on te strucure. Two trucks and five men assited in the response. All units were back in the house by 3:30 pm.

On 10-28-2011 at approx. 4:26 pm D-4 responded to 791 Farmview Rd to the Bob Evans Farm Museum on an Automatic Alarm activation. Upon arrival units found it to be a false alarm. Sytem was reset and all units were back in the house by 4:38 pm.

On 10-18-2011 at approx. 3:41pm. D-4 responded approx. 1 mile out Buckeye Hills Rd on an assist to the OSP for traffic control on a two car MVC with no injuries. All units were back in the house by 4:33 pm.
EMS Assist/ Raccoon Twsp.
On 10-10-2011 at approx. 6:52 pm D-4 responded to the area of SR 588 and Creekview Rd to assist EMS in removing a patient from the woods. Units found a female patient that had been thrown from a horse with a leg injury. Units assisted the patient out of the woods via ATV to the awaiting EMS unit. EMS transported subject to local Hospital for treatment. All units were back in the house by 7:29 pm.
False Alarm/ Raccoon Twsp.
On 10-8-2011 at approx. 7:44 pm D-4 responded to the area of SR325 and US 35 on a report of a vehicle fire. Upon arrival units found that the radiator hose had busted. All units cleared the scene and wer back in the house by 7:53 pm.
Gas Odor/ Raccoon Twsp.
On 10-07-2011 at approx. 6:12 pm D-4 responded to the area of SR 588 and Farmview Rd on a report of strong smell of gas in the area. Upon arrival units found subjects were cutting open old propane tanks. All units cleared the scene and were back in the house by 6:40 pm.
MVC/ Mutual Aid D-7
On 10-06-2011 at approx. 8:28 pm D-4 responded mutual aid to D-7 on a report of a one car MVC with entrapment. Upon arrival, units found there was no entrapment however the patient was located up the road at a residence with severe trauma to the leg. Ems units transported her from the scene to the LZ located at Buckeye Hills Career Center where air medical transported her to a trauma center. All units were back in the house by 9:55 pm.

On 9-28-2011 at approx. 9:43 pm D-4 responded to the area of SR 588 and Autumn Hill Rd on a report of a one car MVC into an embankment. Upon arrival units found the patient out of the vehicle walking around. Units assisted EMS and patient was transported to the hospital. All units were back in the house by 10:16 pm.
Car Fire/ Raccoon Twsp.
On 9-27-2011 at approx. 5:01 pm D-4 responded to 491 Eagle Rd on a report of a vehicle fire. Upon arrival units found the vehicle in question with the fire being out and the owner no were around. Overhaul was performed on the vehicle and all units were back in the house by 5:43 pm.

On 9-25-2011 at approx. 2:53 am D-4 responded to the area of Wolfe Run Rd and Symmes Creek Rd on a report of a MVC over the embankment. Upon arrival the occupant of the vehicle had fled the scene. Scene was turned over to the Ohio State Highway Patrol. All units were back in the house by 4:04 am.
Search/ Village
On 9-24-2011 at approx. 7:38 pm. D-4 responded to the area of 109 SR 325 N with the Rio Grande Police Dept on search of a young juvenile in the area. All units were back in the house by 8:54 pm.
Alarm Activation/ Perry Twsp.
On 9-24-2011 at approx. 4:58 pm D-4 responded to 2095 Wolfe Run Rd on an automatic alarm activation. Upon arrival units found nothing showing. After further investigation it was determined to be a false alarm. All units were back in the house by 5:36 pm.
Electrical Fire/ Mutual Aid with Madison Jefferson FD
On 9-22-2011 at approx. 6:29 pm D-4 responded mutual aid to 981 Hall & Davis Rd on a report of a possible structure fire. Upon arrival units found the air conditioner had caught on fire due to an electrical problem. There was minor smoke damage to the residence and minor damage to the air conditioner. All units were back in the house by 7:04 pm.
Gas Odor/ Raccoon Twsp.
On 9-18-2011 at approx. 1:34 pm D-4 responded to the Bob Evans Farms restraunt on a report of an odor of gas in the area. Upon arrival gas company was already on the scene. Units and the gas company were unable to come up with a source of the odor. Scene was left in control of the gas company. All units were back in the house by 2:11 pm.

On 9-15-2011 at approx. 12:47am D-4 responded to the Bob Evans Farms Restraunt 10854 SR 588 on an alarm activation. Upon arrival units found nothing showing. Upon arrival of keyholder it was determined the alarm was activated by dust in the system from construction work. All units were back in the house by 1:42 am.

On 9-13-2011 at approx. 1:34 am D-4 responded to the University of Rio Grande New Dorm on an automatic alarm activation. Upon arrival units found the activation to be false. All units were back in the house by 1:53 am.

On 9-11-2011 at approx. 7:58 pm D-4 responded to the area of 679 Buckeye Hills Rd on a report of an unknown type fire. Upon arrival units found a controlled burn. All units were back in the house by 8:11 pm.
Assist EMS/ Raccoon Twsp.
On 9-11-2011 at approx. 1243 pm D-4 responded to the area of the BoB Evans Shelter house on a request for assistance from the Gallia County EMS. Upon arrival units assisted the EMS in removing an injured biker from a wooded area were they were having a cross country bike race. The patient was carried out of the woods and transported by EMS to HMC medical center. All units were back in the house by 1:14pm.

On 9-6-2011 at approx. 6:02 am D-4 reponded to the Bob Evans Farms Restraunt 10854 SR 588 on an alarm activation. Upon arrival units found nothing showing at the business. Keyholder arrived on scene and was determined to be a false alarm. All units were back in the house by 6:16 am.

On 9-3-2011 at approx.12:20 pm D-4 responded to the area of 260 Cricket trail on a report of an unknown type fire. Upon arrival units found a controlled burn at the residence. All units were back in the house by 12:57 pm.
Alarm Activation/ Village
On 9-3-2011 at approx. 3:23 am D-4 responded to the University of Rio Grande New Dorm on an alarm activation. Upon arrival units found a washing machine had burnt a belt causing the smoke detector to activate. There was no damage other than the belt on the machine. All units were back in the house by 3:37 am.
Structure Fire/ Mutual Aid to D-12
On 8-11 2011 at approx. 1:02 am D-4 responded mutual aid to 3484 White Rd on a req for water supply. Upon arrival D-12 had a well involved single story structure fire. D-4 assited D-12 by supplying them with approx. 5500 gallons of water. All units were back in the house by 0337 am.
MVC/ Mutual Aid to D-12
On 7-27-2011 at approx. 2:41 pm D-4 responded Mutual Aid to to SR 850 approx a mile from Co. Rd 35 on a report of a MVC rollover with possible entrapment. Upon arrival, patient had been ejected from the vehicle with significant trauma. A landing zone was set up in the lot of the corner stone church of christ. Patient was transported by air medical. All units were back in the house by 4:04 pm.
Power Lines Down / Village
On 7-22-2011 at approx. 4:38 pm. D-4 responded to the area of SR 325 and Richards Ave on a report of power lines down. Upon arrival units found a tree had fallen onto the powerlines. Units secured the scene until the power company arrived. All unit were back in the house by 5:08 pm.
False Alarm / Village
On 7-17-2011 at approx. 8:24 pm D-4 responded to the area of Calvary Baptist Church on a motorist report of a possible brush fire. Units checked the area and could not find anything. All units were back in the house 8:42 pm.
MVC / Raccoon Twsp.
On 7-17-2011 at approx. 11:48 am D-4 responded to the area of Bob Evans Farms on a report of a two vehicle crash. Upon arrival all subjects were out of the vehicles. EMS transported one patient to HMC. All units were back in the house by 1:12 pm.
Gas Odor / M.A. Springfield FD
On 7-14-2011 at approx. 6:38 pm D-4 responded mutual aid to D-12 in the area of Ferrell Gas on the smell of propane in te area. Upon arrival the smell was actually a result of residence in the area cutting open old propane tanks. All units were back in the house by 7:00 pm.
Assist EMS / Raccoon Twsp.
On 7-13-2011 at approx. 9:00 pm D-4 responded to the area of Sailor Rd and SR 325 on a report of a lawn mower accident with possible amputation. Subjects that called the report in transported subject by private vehicle. All units were cancelled and were back in the house by 9:21 pm.
Smoke Scare / Perry Twsp.
On 7-12-2011 at approx. 1:26 pm D-4 responded to 3701 Cherry Ridge Rd on a report of smoke in a structure. Upon arrival units found that the occupant had left a bowl of oat meal in the microwave to long. There was no damage reported and all units were back in the house by 1:40 pm.
Power Line Fire / Raccoon Twsp.
On 7-11-2011 at approx. 10:57 pm D-4 responded to the area of 30 Pleasant Valley Rd. on a report of power lines on fire. Upon arrival units found the three phase service was broken off from the pole and into the trees and burning. AEP arrived on the scene and it was turned over to them. All units were back in the house by 12:31 am.
Automatic Alarm / Village
On 7-11-2011 at approx. 7:20 pm D-4 responded to the University of Rio Grande Lynne Center on an automatic alarm activation. Upon arrival units found someone had pulled a pull station in the east corridor. The alarm was reset. All units were back in the house by 7:36 pm.
Transformer Fire / Perry Twsp.
On 7-11-2011 at approx. 6:22 pm D-4 responded to the area of 3659 centerpoint rd on a report of a transformer fire. Upon arrival power company was on the acene and the fire was out. Units stood by while the power company attempted a reset. All units were back in the house by 7:19 pm.
Lift Assist / M.A. Springfield FD
On 7-5-2011 at approx. 1:00 pm d-4 responded mutual aid to D-12 at 14129 SR 554 on a lift assist. Upon arrival units assisted Life EMS in loading a patient. All unit were back in the house by 2:07 pm.
Brake Fire / Raccoon Twsp.
On 7-5-2011 at approx. 7:33 am D-4 responded to the area of US 35 and SR 325 on a report of a semi on fire. Upon arrival units found the brakes on the trailer had been on fire but had been extinguished with a portable extinguisher prior to our arrival. Units overhauled the area and left the scene in control of OSP. All units were back in the house by 7:59 am.
Rubbish Fire / Perry Twsp.
On 6-21-2011 at approx. 5:40 pm. D-4 responded to the area of Patriot Rd and Grate road on a report of a structure fire. Upon arrival units found a rubbish fire back off the roadway. The scene was made safe and all unit were back in the house by 6:39 pm.
Lift Assist / Perry Twsp.
On 6-18-2011 at approx. 3:56 pm. D-4 responded to 3424 SR 775 on a request for lifting assistance from Gallia County EMS. Upon arrival units assisted the EMS in loading the patient who was having cardiac problems. All units were back in the house by 5:00 pm.
Alarm Activation / Perry Twsp.
On 6-15-2011 at approx. 9:28 pm D-4 responded to 1860 Patriot Rd. on an automatic fire alarm. Units were canceled enroute by the alarm company reporting this as false alarm. All units were back in the house by 9:31 pm.
Lift Assist / M.A. Springfield FD
On 6-10-2011 at approx. 0959 am D-4 assisted D-12 and Gallia County EMS at 14129 SR 554 on lifting assistance of a patient. All units were back in the house by 11:11 am.
Chimney Fire / M.A. Gallipolis FD
On 6-4-2011 at approx. 11:46 am D-4 responded Mutual Aid to D-6 at 1951 Cora Mill Rd on a report of a structure fire. Upon arrival units found that it was a chimney fire. Units extinguished the fire and made the chimney safe. All units were back in the house by 1:19 pm.
MVC / M.A. Springfield FD
On 6-3-2011 at approx. 0252 am D-4 responded Mutual Aid with D-12 on a report of a MVC with possible entrapment in the area of SR 850 and CO. Rd. 35. Upon arrival all patients were out of the vehicle. Patients were transported by private vehicle to Holzer Medical Center. All units were back in the house by 03:21 am.
Controlled Burn / Perry Township
On 5-27-2011 at approx. 3:05pm D-4 responded to the area of 1617 Patriot Rd on a report of a tire fire. Upon arrival only thing units could find was a small controlled burn were a subject had burnt a piece of furniture. Occupants were advised of the burning regulations and all units were back in the house by 3:34pm.
CO Alarm / Village
On 5-25-2011 at approx. 10:39pm D-4 responded to 152 Lake Dr. on a report of a Carbon Monoxide alarm. Upon arrival units tested the air quality with a CO meter and found no readings or evidence of CO in the residence. Units determined the detector was probably malfunctioning and disconnected the alarm as well as turn the gas off to the residence til it could be checked out further by a technician. All units were back in the house by 11:22pm.
Search / Village
On 5-25-2011 at approx 09:57am D-4 responded to the Rio Grande Village park to assist the Rio Grande Police Department in conducting a possible search for a male subject. Upon arrival all units were canceled by the Police due to the search was not needed at this time. All units were back in the house by 10:32am.
Car Fire / US 35 Westbound Lane
On 5-24-2011 at approx. 5:11am D-4 responded to the area of US 35 and Raccoon Creek on a report of a car fire. Upon arrival units found the vehicle fully involved. Units extinguished the fire with an 1 3/4 handline and vehicle was towed from the scene. D-4 received assistance from the Springfield Twsp FD due to original call was unsure what territory the fire was in. OSP was also on scene. All units were back in the hours by 06:02am.
MVC Entrapment / M.A. to Springfield FD
On 5-22-2011 at approx. 7:45pm D-4 responded mutual aid with Springfield Twsp. FD to the intersection of County Rd 35 and SR 850 on a two car MVC with entrapment. Upon arrival units performed an extrication on one of the vehicles then assisted EMS with the patients. All patients were transported to Holzer medical center with moderate to severe injuries. All units were back in the house by 8:49pm.
Structure Fire / Raccoon Twsp.
On 5-19-2011 at approx. 08:38 am D-4 responded to 671 Gooch Rd. on a report of a structure fire. Upon arrival units found the residence full of smoke. Units found that there had been a fire in the bathroom of the residence but was self extinguished by water from equipment that was in the bathroom. The bathroom was overhauled and smoke removed from the residence. There were no injuries. All units were back in the house by 09:40am.
Dumpster Fire / Village
On 5-7-2011 at approx. 10:23 am D-4 responded to 45 Richards St. on a dumpster fire. On arrival units found a small fire in the dumpster. Rio Grande PD was on scene and had empty an Extinguisher on the fire. Units finished extinguishing the fire. All units were back in the house by 10:44 am.
Search / Perry Twsp.
On 4-25-2011 at approx. 8:56 pm D-4 responded to 4352 Cora Mill Rd. on a report of an elderly male with Alzheimer that had went into the woods. Family members found the male subject prior to our arrival. All units were canceled and back in the house by 9:22 pm.
Stove Fire / Village
On 4-23-2011 at approx. 5:03 pm D-4 responded to the area of North Atwood and Lewis Ave. on a report of an oven fire. Upon arrival fire was out. Units secured the oven and made the scene safe. All units were back in the house by 5:17 pm.

On 4-20-2011 at approx. 1:47pm D-4 responded with the VInton FD to the area of Mt tabor Rd and Sailor Rd on a report of a structure fire. Vinton had requested D-4 for a tanker. All units were back in the house by 3:52
Gas Leak / Village
On 4-19-2011 at approx. 12:30 pm D-4 responded to the area of East College and North College a report of a gas line leak at a construction site. Upon arrival unit found a gas main had been struck . Units secured the scene until gas company arrived. All units were back in the house by 1:30.
Structure Fire / M.A. Gallipolis FD
On 4-15-2011 at approx. 8:58 pm D-4 responded to a mutual aid request from the Gallipolis FD on a structure fire with possible entrapment at the Buckridge apartments. Gallipolis request assistance of manpower and tankers. Upon arrival all units were staged and released a short time later. There turned out to be no entrapment and fire was knocked down in a short time. All units cleared the scene and were back in the house by 9:39 pm.
False Alarm / Raccoon Twsp.
On 3-28-2011 at approx. 2:28 pm. D-4 responded to the area of the Raccoon Creek Bridge and SR 588 on a report of a brush fire. Upon arrival the fire turned out to be a controlled burn. All units cleared the scene and were back in the house by 2:43 pm.
Downed Lines/ Village
On 3-23-2011 at approx. 5:26 pm D-4 responded to the intersection of SR 325 and SR 588 on a report of power lines down. Upon arrival units found that high winds from a passing storm had downed the caution lines at the intersection. Units stood by until the lights could be reenstalled. All units were back in the house by 6:56 pm.
MVC Entrapment/ Perry Twsp.

On 3-22-2011 at approx. 8:20 am D-4 responded to the area of 3020 SR 775 on a report of a MVC with entrapment. Upon arrival units found a single vehicle crash of a SUV that had hit a tree. Units found one occupant in the vehicle with injuries and some minor entrapment. Units removed part of the vehicle with the JAWS of LIFE to enable better packaging of the patient by EMS personnel. Patient was transported from the scene to the Raccoon Creek County Park were she was transfered to an air medical helicopter. Healthnet transported patient to a Hospital in Huntington. OSP, Gallia County EMS, and the Gallia County Rescue were also on scene . All units were back in the station by 9:41 am.
MVC/ Perry Twsp.
On 3-12-2011 at approx. 7:00 pm D-4 responded to the area of Cora Mill Rd. and Creekview Rd. on a report of a MVC that possibly ready to go into the creek. Upon arrival units found a full size van impaled into a guardrail and leaning toward the flood waters. There were no occupants inside of the vehicle or in the area. It was later determined the occupant walked down the road to his house and was uninjured. EMS units were canceled and the scene was turned over to the State Highway patrol. All units were back in the house by 7:40 pm.

On 3-6-2011 at approx. 9:26 pm D-4 responded to the area of 86 Gooch Rd. on an unknown type of alarm. Units were staged in the area until law enforcement coulgd arrive do to the unknown nature of alarm. All units were canceled with the alarm being a burglary alarm. Scene was handle by the gallia county Sheriffs dept. All units were back in the house by 11:21pm.

On 3-4-2011 at approx. 4:51 pm D-4 responded to the area of 2580 Patriot Rd. on a report of a brush fire. Upon arrival units found a field on fire that covered approx. 3/4 of an acre. Units extinguished the fire and were back in the house by 5:34.pm

On 3-4-2011 at approx. 3:51 pm D-4 responded to a Mutual Aid call with the Madison Jefferson FD on a working structure fire on Thurman Rd. Upon arrival units assisted in water supply and manpower. All units were back in the house by 6:25 pm.
MVC Rollover / MA Vinton FD
On 3-2-2011 at approx. 7:22 pm D-4 responded Mutual Aid to D-7 to a MVC Rollover with report of entrapment in the area of 1373 Sailor Rd. Upon arrival units found a single vehicle on it's top with all occupants out of the vehicle. All patients refused treatment from the Gallia County EMS and the scene was turn over to the Vinton FD. All units were back in the house by 7:54 pm.
Chimney Fire / Perry Township
On 2-22-2011 at approx. 11:05 am, D-4 responded to 14325 SR 141 near the intersection of SR 233 on a report of a chimney fire. Upon arrival a portable extinguisher had been used on the fire. The owner requested from D-4 that he did not need any other assistance and canceled us on the scene. 4 Trucks and 9 Men responded. All D-4 units were back in the house by 11:50 am.
Structure Fire / Village
On 2-19-2011 at approx. 2:13 am, D-4 responded to 401 Central Ave. on a report of a structure fire. Upon arrival units found heavy smoke coming from the basement and first floor area of the structure. Units brought the fire under control within 20 minutes. There was moderate fire damage to the basement and part of the first floor with extensive smoke damage to the rest of the structure. There was no one home at the time of the fire and the cause has yet to be determined. 19 men and 4 trucks responded. Gallia County EMS was on the scene for standby. All units were back in the house by 5:52 am.
Brush Fire / Perry Twsp.
On 2-17-2011 at approx. 2:03 pm, D-4 responded to the area of 1771 Centerpoint Rd. on a report of a brush fire. Upon arrival units found approx. 2 acres of wooded area and brush on fire. Units had the fire under control within 30 minutes. Two trucks and seven men responded. All units were back at the house by 3:51 pm.
MVC Entrapment / Perry Twsp.
On 2-14-2011 at approx. 10:37 am, D-4 responded to the area of 3835 SR 325 S on a report of a 3 vehicle MVC with entrapment. Upon arrival units found one vehicle with occupants still inside the vehicle. Units had to use the JAWS of Life to remove the drivers side door to free the occupants. Occupants of the other two vehicles received minor injuries and were out walking around. Three patients were transported by EMS to the hospital. The road was closed for approx. 2 hrs while the OSP conducted there investigation. There were 2 trucks and 12 men on scene and all units were back in the house by 12:47 pm.

On 2-13-2011 at approx. 9:25 pm, D-4 responded to the area of Creekview Rd just off SR 588 on a report of a one car MVC rollover. Upon arrival units found a vehicle on it's top with the occupant out and walking around. Units on scene secure the vehicle and assisted EMS units on the scene. The occupant was not injured and the scene was turned over to the Gallia county Sheriffs Office. There were 18 men and 2 trucks responded and all units were back in the house by 10:05 pm.

On 2-12-2011 at approx. 11:08 am, D-4 responded to Cherry Ridge Rd on a lift assist with Life Ambulance Service. Units on scene assisted in loading the patient and were back in the house by 11:43 am.

On 2-9-2011 at approx. 5:36 pm, D-4 was requested by Gallia County Sheriff Joe Browning to assist in a search of an elderly male subject with Alzheimer that was missing.The missing gentleman had left his residence in a vehicle with an unknown direction of travel. D-4 traveled various roads in Perry Township during the search process and was unable to locate the victim. The victim was later located in Jackson County by the Jackson City Police Department.The victim was ok and released back into the care of his family. All units were back in the house by 7:02 pm.

On 2-5-2011 at approx. 10:38 pm, D-4 responded to the New Dorm at the University of Rio Grande on an alarm activation. Upon arrival units found that an unknown person had pulled the alarm on the 2nd floor. Campus pd restored the system. Two trucks and 10 men responded. All units were back in the house by 10:47.

On 2-4-2011 at approx. 10:57 am, D-4 responded to Wood Hall at the University of Rio Grande on an alarm activation. Upon arrival units found that the smoke detector in the mechanical room had been activated. Upon investigation the detector had been activated due to dust created from a shop vac. Two trucks and 10 men responded. All units were back in the house by 11:10 am.
MVC Rollover/ Raccoon TWSP.
On 2-2-2011 at approx. 8:04 pm D-4 responded a mile North of US 35 on SR 325 on a report of a one car rollover. Upon arrival units found the patient was still in her vehicle. Units assisted EMS personnel in removing the patient from the vehicle and EMS transported the patient to the hospital. A patch of ice on the road was the apparent cause of the crash. Two trucks and 16 men responded and all units were back in the house bty 9:18 pm.

On 1-22-2011 at approx. 6:50 pm D-4 responded to 2260 Scenic Dr. The Vinton Fire Dept. requested mutual aid for tanker support on a working structure fire. D-4 responded with T-45 and later also sent T-40 for air supply. The situation was contained and all units were back in the house by 9:51 pm.

On 1-20-2011 at approx. 3:15 am D-4 responded to the area of H. Paul Ave and SR 325 South on a report of a fire and explosion. Upon arrival units found a box truck being used for storage was on fire. Fire was extinguished and all units were back in the house by 5:33 am.

On 1-13-2011 at approx . 2:37 pm D-4 responded to the Bob Evans Restaurant on a smell of gas in the building. Upon arrival units checked the building with gas detectors and found no traces of a combustible product. The scene was turned over to the manager and all units were back in the house by 3:24 pm

On 1-9-2011 at approx. 4:44 pm D-4 responded to 351 Buckeye Hills Rd on an alarm activation. Upon arrival units found a busted sprinkler line in building ( B ). Units valved the dry system off and released the scene to the key holder. All units back in the house by 5:37 pm
Chimney Fire / Perry Twsp.
On 1-8-2011 at approx. 6:29 am D-4 responded to 720 SR 233 on a report of a chimney fire with possible structure involvement. Upon arrival units found some fire still in the chimney with no structural involvement. Units used a dry chem extinguisher to put the fire out. Due to the initial call and the snowy road conditions D-3 was requested to respond for mutual aid. D-3 was canceled en-route and all other units were back in the house by 7:39 am.

On 12-17-2010 at approx. 01:30 pm D-4 responded to 92 Ruth Rd to assist EMS with a medical emergency. Upon arrival units had to take apparatus back in to an area of road that the EMS could not access due to snow conditions. D-4 transported patient out to the awaiting EMS unit were they then transported the patient to the hospital. All units were back in the house by 2:36 pm.

On 12-8-10 at approx. 10:30 am D-4 responded to 1570 Patriot Rd on a report of an electrical box fire. Upon arrival units found fire in the wall around the electrical box. Fire was extinguished and power disconnected from the structure. Buckeye rural electric was called to the scene. All units were back in the house by 11:45 am.

On 12-8-10 at approx. 7:56 am D-4 responded to the area of Bob Evans Farms on a report of a two car MVC. Upon arrival units assisted in traffic control for the Ohio State Patrol. EMS was on the scene and checked out one patient but did not transport the subject. The scene was turned over to OSP and all units were back in the house by 8:33 am.

On 12-8-10 at approx. 12:02 am D-4 responded to the Holzer Hall Dorm at the University of Rio Grande on a report of light smoke in the building. Upon arrival units were unable to locate any smoke or source of smoke. Building was checked due toi electrical problems in the area and were unable to locate anything. Alll units were back in the house by 12:16 am.

On 12-3-10 at approx. 8:20 am D-4 responded to a report of a structure fire at 1636 Cherry Ridge Rd. Upon arrival units found heavy fire conditions in the east side of the structure. Units brought the fire under control within 30 min and were assisted on the scene by units from D-5 and D-3. One minor injury was reported to an occupant and was treated at the scene by EMS personnel. State fire marshal was called in to investigate even though the fire does not appear to be suspicious. All units were back in the house by 1:35 pm.

The fire was out on arrival due to the the fire burning through one of the water lines. D-4 received mutual aid from D-3. All units were back in the house by 8:30 am.

Upon arrival EMS units advised for the FD to set up a Landing Zone for Air Medical at the Raccoon Creek county park. Units assisted EMS with loading patient into the helicopter and were back in the house by8:33 pm.

On 11-8-2010 at approx. 3:37 pm D-4 responded to the Rio Grande Village park on a report of a brush fire. Upon arrival the fire had been put out by the Rio Grande Police Department. Units mopped up the area and were back in the house by 3:56 pm.

On 11-5-2010 at approx. 6:16 pm D-4 responded to the area of SR 588 and Autumn Hill Rd on a report of a car Vs. Amish Buggy. Upon arrival unit assisted EMS personnel in packaging patients for transport. All patients received non life threatening injuries and were transported to Holzer Medical center. All units were back in the house by 7:41 pm.

On 11-3-2010 at approx 11:26 pm D-4 responded to 90 Gavin St in Rodney Village II on a report of a working structure fire. D-4 was requested by the Springfield Twsp. FD for mutual aid. D-12 requested manpower and apparatus. Upon arrival there was heavy fire already coming through the roof. Units made an interior attack and brought the fire under control within 15 min. All units were tide up on the scene for approx 1 1/2 hrs. There were no injuries and there was extensive damage to the structure. D-4 units were back in the house by 1:24 am.

On 11-2-2010 at approx. 5:04 pm D-4 responded to 117 Lake Dr. on a report of a brush fire. Upon arrival units found a small grass fire in a field area. Fire was extinguished and all units were back in station by 5:29 pm.

On 10-24-2010 at approx. 3:09 pm D-4 responded to the area of 1156 SR 325 South on a report of a one vehicle MVC rollover. Upon arrival units found the vehicle off the roadway and on its top. Patient was out of the vehicle and refused treatment from EMS personnel. All units were back in station by 3:35 pm.

On 10-23-2010 at approx. 4:41 pm D-4 responded to the area of 374 Skyline Rd. on a report of a fully involved trailer fire. Upon arrival units found a single wide trailer fully involved. Multiple handlines were used to extinguish the blaze. Upon investigation the cause was determined to be accidental in nature. There was one minor civilian injury which did not require transport. All units were back in station by 7:12 pm.

On 10-22-2010 at approx. 6:47 am D-4 responded to the area of the College Hill Motel on a report of a vehicle fire. Upon arrival units found a vehicle that had hit a deer and then caught fire. Units found moderate fire coming from the engine compartment and extinguished it with a handline. Scene was turned over to the State Patrol. All units were back in station by 7:12 am.

On 10-21-2010 at approx. 4:50 pm D-4 responded to a Mutual Aid request from the Gallipolis FD to the area of Oliver Rd and Addison Pike. D-6 requested manpower and brush apparatus on a large brush fire. Units were on the scene for several hours and returned to the station by 9:35 pm.

On 10-16-2010 at approx. 11:27 pm D-4 responded to the area of 468 Pleasant Valley Rd. on a report of a one car MVC rollover. Upon arrival units found two patients that had been ejected from the vehicle with one patient in critical condition. Units set up a Landing Zone for the EMS at the Buckeye Hills Career Center and assisted EMS in patient transport. One patient was transported by air medical and the other was transported by ground. All units were back in station by 12:16 am.
ATV Accident/ Raccoon Twsp.
On 10-16-2010 at approx 2:49 pm D-4 responded to the area of 49 Edoma trail on a report of an ATV Accident. Upon arrival units found one victim of the accident. Units assisted the Gallia County EMS in packaging the patient. EMS transported the patient to the hospital. All units were back in station by 3:56 pm.

On 10-16-2010 at approx. 1:07 pm D-4 responded to the URG New Dorm on an Automatic Alarm activation. Upon arrival units found the alarm to be false. All units were back in station by 1:26 pm.

On 10-11-2010 at approx. 4:47 pm D-4 responded to the area of 138 Pine St. on a report of a brush fire. Upon arrival units found a small area of brush and debris on fire. Units extinguished the fire with a minimal amount of water. Upon investigation units learned the fire was set by several juveniles in the area. Information was passed on to the Rio Grande Police who later found the suspects. All units were back in station by 5:04 pm.

On 10-11-2010 at approx. 11:31 am D-4 responded to the area of 524 Fellure Rd. to set up an Landing Zone for the Gallia County EMS. Upon arrival units set up the LZ in a large open field. Units assisted EMS in packaging the patient for transport. All units were back in station by 1:04 pm.

On 10-11-2010 at approx. 1:20 am D-4 responded to the area of Wolf Run Rd and SR 325 on a reort of a one car MVC with entrapment. Upon arrival units found a vehicle on it's side in a ditch with one occupant in it. Units assisted the victim out of the vehicle through the rear glass. Victim refused treatment from EMS personnel and the scene was turned over to the State Patrol. All units were back in station by 1:57 am.
False Alarm/ Perry Twsp.
On 10-3-2010 at approx. 5:49 pm D-4 responded to 5665 Cora Mill Rd. on a report of a possible gas leak in the residence. Upon arrival units turned off the gas to the residence and checked the residence with a gas meter. There was no evidence of any type of gas in the residence. Gas was left off to the residence and the occupant was transported to the hospital for an unrelated breathing problem. All units were back in the station by 6:32 pm.

On 9-24-2010 at approx. 5:02 pm D-4 responded to 2309 SR 325 South on a report of an automatic alarm activation in a residence. Upon arrival it was found that the alarm company was working on the system. All units were back in the house by 5:15 pm.

On 9-24-2010 at approx. 4:11 pm D-4 responded to 1128 Vanco Rd on a mutual aid request from the Gallipolis Fire Department. Gallipolis was on the scene with a hay bailer and brush fire. Gallipolis requested water and manpower to the scene. D-4 was on the scene for approx. 2 1/2 hrs. All D-4 units were back in the house by 7:10 pm.
Electrical Fire/PerryTwsp.
On 9-22-2010 at approx. 9:00 pm D-4 responded to 1526 Centerpoint Rd. on a report of an electrical fire. Upon arrival there was nothing showing from the residence. Upon further investigation there was no fire found. The electrical box in the house experienced a mechanical malfunction. The scene was turned over to the homeowner and all units were back in the house by 10:35 pm.
Barn Fire/ M.A. to D-3

On 9-13-2010 at approx 8:21 am D-4 responded to a mutual aid request from the Greenfield Twsp. FD on a report of a barn fire at 61 Flagspring Rd. Upon arrival units advised of a barn fire with over 250 round bales of hay with exposure problems. D-4 assisted in providing water for the exposures. All D-4 units were back in station by 11:02 am. D-3 remained on scene until approx.7:18 pm dealing with exposure issues. There were no injuries

Upon arrival units found that a pickup truck and a tractor trailer had sideswiped each other. There were no injuries. Units provided traffic control and the scene was turned over to the Ohio State Highway Patrol. All units were back in station by 10:39 pm.

On 8-9-2010 at approx. 3:32 pm D-4 responded to an automatic alarm activation at 2095 Wolfe Run rd. Upon arrival units found the alarm to be false in nature. All units were back in station by 4:13 pm.

On 8-3-2010 at approx. 11:52 am D-4 responded to the Family Dollar store on a report of a car fire. Upon arrival the fire was out. Vehicle was overhauled and all units were back in station by 12:09 pm.

On 8-3-2010 at approx. 11:11am D-4 responded to the Rio Grande Day Care on an automatic alarm activation. Upon arrival units were canceled due to steam from cooking setting off alarm. All units were back in station by 11:14 am.

On 7-22-2010 at approx 12:54 pm D-4 responded to the University of Rio Grande New Dorm on an automatic alarm activation. Upon arrival units found the alarm to be false in nature. All unit were back at station by 1:02 pm.
Brush Fire /Perry Twsp.
On 7-21-2010 at approx. 2:55 pm D-4 responded to 122 Patriot Rd on a report of a brush fire. Upon arrival units found the brush fire to be minor in nature, however units found a female that had received burns to the upper portion of her body. Gallia County EMS responded to the scene and patient was transported to the hospital by Medical Helicopter. D-4 assisted in setting up the LZ for the helicopter. All units were back in station by 4:46 pm.

On 7-16-2010 at approx. 10:26 am D-4 responded to the intersection of East College St. and SR 588 on a report of a vehicle fire. Upon arrival units found a vehicle with fire involvement in the engine compartment. Unit extinguished the fire without incident. All units were back in station by 11:06 am.

Assist Sheriffs Office / Perry Twsp.
On 7-6-2010 at approx. 11:01 am D-4 was requested to standby for for the Sheriffs Office while the SWAT team made entry into a residence on Brushy Point Road. After they made entry D-4 was requested to ventilate the structure of tear gas. All units were back in station by 3:33 pm

On 6-29-2010 at approx. 7:20 pm D-4 responded to 160 Dorsey Drive on a report of an automatic fire alarm. The alarm was do to a malfunction of the system. All units were back in service by 7:33 pm.
Gas Leak / Village
On 6-29-2010 at approx 5:47 pm D-4 responded to the area of 430 Lake Drive on a report of a gas leak. Upon arrival units found that a construction crew had cut into a 2" gas line with a backhoe. The area was secured and Columbia Gas was contacted. All units were back in station by 8:00 pm.
LZ set up / Village
On 6-25-2010 at approx. 1:56 am D-4 responded to the Bob Evans Farms Hall parking lot to set up an LZ for medflight 3. Gallia County EMS had an 11 yo patient with a possible head injury. All units were back in station by 2:46 am.

On 6-24-2010 at approx. 12:09 PM D-4 responded to 17936 SR 160 on a report of a structure fire. The Vinton Fire Dept. had requested mutual aid for a truck and manpower. All units were back in station by 1:24 pm.

On 6-17-2010 at approx. 6:15 pm D-4 responded to the area of SR. 141 and Gage.Upon arrival units found a singal vehicle MVC that was over an embankment. There was one injury and no entrapment. Units assisted the Gallia County EMS in bring the subject up over the hill. All units were back in station by 7:11 pm.

On 6-12-2010 at approx. 6:49 pm D-4 responded to 229 Bandy Rd. to Assist the Gallia County EMS in transporting a patient out of the woods. All units were back in station by 7:34 pm.
CO Alarm / Village
On 6-6-2010 at approx. 10:43 pm D-4 responded to the Rio Grande Estates on a report of a Carbon Monoxide alarm going off. Upon arrival unit found the detector to have been set off from smoke from an outside fireworks display. All units were back in station at 11:00 pm.
MVC Entrapment / Raccoon Twsp.
On 5-21-2010 at approx. 7:19 am D-4 responded to the area of Bob Evans restaurant in Rio Grande on a report of a three car MVC with possible entrapment. Upon arrival units found a three vehicle rear end collision with entrapment in the rear passenger area of one of the vehicles. Units on scene used the Jaws of Life to make entry into the door of the vehicle then assisted EMS personnel in packaging the patients for transport. Two patients were transported by Gallia County EMS to Holzer Medical Center. All units were back in station by 8:02am.

On 5-9-2010 at approx. 11:17 pm D-4 responded to the University of Rio Grande New Dorm on an automatic alarm activation. Upon arrival units found the alarm to have been caused by a malicious act. The scene was turned over to campus police and all units were back in station by 11:33 pm.

On 5-06-2010 at approx. 10:46 pm D-4 responded to 3607 Garners Ford Rd. on a report of a structure fire at the S&J Lumber Company. Upon arrival units found fire in the back of the structure. Units attacked the fire with two hand lines and brought the fire control in a very short time. Units were on the scene for approx 3 three hours controlling hot spots. There were no injuries reported. All units were back in station by 1:36 am.

On 5-03-2010 at approx. 6:56 pm D-4 responded to the Davis Hall dormitory on an automatic fire alarm activation. Upon arrival units found that the alarm had been activated by a burnt bag of popcorn. All units were back in station by 7:04 pm.

On 5-03-2010 at approx. 6:27 am D-4 responded to 5067 Cora Mill Rd. on a report of water getting into a residence from flood waters. Upon arrival units found that the water had made it up into the residence yet. Units were back in station by 7:09 am.
Water Rescue / Perry Twsp.
On 5-02-2010 at approx. 10:42 D-4 responded to the area of SR 325 South and Cora Mill Rd. on a report of two vehicles stuck in high waters. Upon arrival units found that all occupants had already freed themselves. Units stood by until ODOT placd high water signs in the area. All units were back in station by 11:29 pm.
Gas Leak / Raccoon Twsp.
On 5-2-2010 at approx. 10:24 pm D-4 responded to the area of 1603 Pleasant Valley Rd. on a report of an LP tank leaking. Upon arrival units found an LP tank had been overturned do to flash flooding and was leaking gas. Units shut the gas off at the valve and contained the situation. Units went back in service at 10:41 pm.
Electric Lines Down/ Village
On 5-01-2010 at approx. 11:07 pm D-4 responded to 11263 SR 588 on a report of power lines down and tree fire. Upon arrival units found that the downed power lines had caught a tree on fire. There was no immediate danger to the residence. AEP was contacted and units stayed on the scene until AEP arrived. All units were back in station by 11:57 pm.
MVC / Perry Twsp.
On 5-01-2010 at approx. 12:30 pm D-4 responded to SR 141 near Lankin Rd. on a report of a one car MVC with possible entrapment . Upon arrival units found one victim with no entrapment but serious injuries. D-4 assisted the Gallia County EMS in packaging the patient and set up a LZ for Medflight. Patient was transported by air to the hospital. All units were back in station by 1:50 pm.

On 4-24-2010 at approx 1:49 pm D-4 responded to 791 Farmview Rd. on an automatic alarm activation. All units were canceled enroute by the alarm company reporting it as a false alarm. All units were back in station by 1:54 pm.
Brush Fire / Mutual Aid to D-3
On 4-22-2010 at approx. 3:01 pm D-4 responded to the area of 7227 Symmes Creek Rd. on a report of a brush fire. The Greenfield Twsp. FD requested mutual aid from D-4 for manpower and a brush unit. Upon arrival D-4 units assisted D-3 in con trolling the fire. There were no injuries and all trucks were back in station by 6:22 pm.
Car Fire / Village of Rio Grande

On 4-21-2010 at approx. 8:32 am D-4 responded to the BP station on a report of a car on fire in front of the gas pumps. Upon arrival bystanders had extinguished the fire with a portable extinguisher. There were no injuries and all units were back in station by 8:56 am.
Structure Fire / Mutual Aid to D-12
On 4-20-2010 at approx. 7:51 am D-4 responded to 230 Hemlock Rd. on a report of a structure fire. Springfield Twsp. FD requested mutual aid from D-4 for manpower. Upon arrival D-12 had the situation contained. All D-4 units were released and were back in station by 8:32 am.
Fatal Structure Fire / Raccoon Twsp.

On 4-18-2010 at approx. 3:09 pm D-4 responded to 1824 Pleasant Valley Rd on a report of a possible structure fire. While enroute to the scene dispatch advised all units that there was possible entrapment inside the residence. Upon arrival units found heavy smoke and fire conditions on the second story of the structure. Mutual aid was requested and an aggressive primary search and interior attack were conducted . The fire was brought under control within 15 min. There were two minor injury's reported of firefighters on the scene and the occupant of the residence did perish in the blaze. Units on the scene were T-41,T-44, T-45 , T-40, D-5, D-7, D-12, Gallia County EMS, Gallia County Sheriff Office, Gallia County Coroner and the State Fire Marshal.All unit were back in station by 7:50 pm. On 4-18-2010 at approx. 11:55 am D-4 responded to 5067 Cora Mill Rd. on a report of a residential fire alarm. While enroute to the scene the alarm company call back with a cancellation. All units were canceled and were back in the station by 12:02 pm. Units that responded were T-41 and T-44.

On 4-15-2010 at approx. 4:17pm D-4 responded to the area of 732 Mudsoc Rd on a mutual aid request from the Harrison Twsp Fire Dept. Upon arrival units assisted in controlling a large brush fire. Units on the scene were D-11, D-3, D-4 and the Wayne national forest. There were no injuries and all units were back in station by 8:54pm.

On 4-12-2010 at approx 2:00pm D-4 responded to 1005 Kokeen Hollow Rd on a report of a brush fire. The Greenfield Twsp. Fire Department requested mutual aid assistance of manpower. T-47 reponded to the scene to assist. All units were back in station by 3:59pm.
MVC / Perry Twsp
On 4-10-2010 at approx 6:24pm D-4 responded to the intersection of Centerpoint Rd and Symmes Rd on a report of a vehicle over an embankment that was smoking. Upon arrival units found a vehicle over the bank however there was no fire. Units assisted EMS with getting the patient loaded up and EMS transported too the hospital. Units on scene T-41,T-47, EMS and OSP. All units were back in station by 7:11pm.
Rollover Over MVC / PerryTswp.
On 4-10-2010 at approx 1:46pm D-4 responded to 1104 SR 141 on a report of a rollover MVC with entrapment. Upon arrival all occupants were out of the vehicle and there was one minor injury. Victim was transported by private vehicle to the hospital. Units on the scene were T-41, OSP and Gallia County EMS. All units were back in station by 2:27pm.

On 4-08-2010 at approx. 6:20pm D-4 responded to car fire in front of the fire station. Upon arrival units found fire in the engine compartment of the vehicle. Units extinguished the blaze with one hose line. Units on the scene were T-41 and Rio Grande PD. There were no injuries. All units were back in station by 7:00pm.

On 4-4-2010 at approx 9:38pm D-4 responded to the area of Buckeye Hills Career Center on a report of a possible brush fire. Upon arrival units found the fire to be controlled in nature. No action was taken and all units were back in station by 9:49pm.

On 4-03-2010 at approx 12:30pm D-4 responded to 3074 Cherry Ridge Rd. on a report of a brush fire. Upon arrival approx an 1/8 of an acre was on fire behind a residential trailer. Due to very high wind conditions the fire was moving quite rapidly into the wooded area. Units were able to make an aggressive attack on the fire and keep it from going any further in to the woods. There were no injuries reported. Units on the scene were T-41, T-47 and T-44. All units were back in station by 1:55pm.
Dumpster Fire / Vilage of Rio Grande
On 3-27-2010 at approx. 3:17pm D-4 responded to 71 Ridge Ave on a report of a dumpster fire endagering a structure. Upon arrival units found a dumpster on fire with no involvement to the structure. The dumpster was extinguished by fire personnel. Units on scene were T-41 and T-44. All units were back in station by 3:38pm.
Electric Pole Fire/ Raccoon TWSP.
On 3-26-2010 at approx. 6:26am D-4 responded to 412 Cheesebrew Rd on a report of an electric pole on fire. Upon arrival units found the top of the pole had burnt into. AEP was contacted to respond to the scene. Units on scene were T-41 and T-47. All units were back in station by 7:53am.
Structure Fire /Village of Rio Grande
On 3-23-2010 at approx. 11:45am., D-4 responded to Rio Estates Apartments on a report of a structure fire with heavy smoke showing. On arrival units found lite smoke coming out of apartment 32. Upon investigation units found that the fire had been extinguished with a portable fire extinguisher. Positive pressure ventilation was established to purge the apartment of remaining smoke. Fire damage was contained to the washroom area. The State Fire Marshal was called in and has ruled the fire to be accidental in nature. There were no injuries. Units on the scene were T-41, T-44, T-45 and the Rio Grande Police Department. All units were back in station by 2:14pm.

On 3-19-2010 at approx. 4:08pm D-4 reponded to 1908 Smokey Row Rd on a large brush fire. The Harrison Twsp. Fire Department requested Mutual Aid for manpower. All units were back in station by 6:4pm.
Structure Fire/ Mutual Aid to D-6

On 3-13-2010 at approx 3:49pm D-4 responded to 381 Buck Ridge Rd. on a report of a working apartment fire. The Gallipolis FD requested Mutual Aid for tanker support. On arrival Gallipolis FD had made an aggressive attack on the fire containing it to the apartment of origin. Two other apartments received some smoke and there was one minor civilian injury. Agencies on the scene were Gallipolis FD, D-4 ( T-45 & T44 ), Gallia County Sherrifand Gallia County EMS. All d-4 units were back in station at 5:06pm.
Structure Fire/ Mutual Aid D-12
On 3-04-2010 at approx 5:35pm D-4 responded to 1827 Prospect Church Rd. Springfield Township for a working structure fire. The Springfield Township Fire Department requested Mutual Aid for water and manpower. Units on the scene were T-45 and Vinton Fire Department personnel. There were no reports of injuries. D-4 units were back in station by 7:19pm.
Gas Leak / Village of Rio Grande

On 3-03-2010 at approx 12:08 pm D-4 responded to a report of a natural gas leak at the intersection of Bevo Francis Way and East College St. On arrival units found that while working on a water main break a backhoe cut into a gas line. Seven buildings were evacuated on the campus of the University of Rio Grande as a precaution. Columbia gas responded to the scene and shut off the gas allowing students to resume classes around 3:30 pm. There were no injuries reported. All D-4 units were back in station by 3:05pm
Structure Fire / Raccoon TWSP.
On 2-25-2010 at approx. 2:47am D-4 responded to 127Wayne Ln. on a report of a structure fire. Upon arrival units found fire in the wall behind a wood burner. Members made access into the wall and extinguished the fire. Fire damage was contained to the wall of origin, with some minor smoke to a portion of the structure. There was no injuries. Units on scene were T-41, T-44 and T-45. All units were back in station at 423am.
Multiple Vehicle MVC /Raccoon TWSP.

On 02-15-2010 at approx. 13:12 D-4 responded to a a multiple vehicle MVC on US 35 approx. 1mile east of Rio Grande. Upon arrival D-4 found several vehicles in multiple areas of the road due to icy road conditions. There was no injuries and no hazards found. Scene was released to OSP and all units returned to quarters. Apparatus on the scene were T-41 and T-40.
Barn Fire / Mutual Aid with Madison Jefferson FD


On 2-10-2010 at approx. 09:38 D-4 responded to a mutual aid request from the Madison Jefferson FD on a working barn fire located on Jimes Emory Rd in Jackson County. D-4 sent two tankers and manpower to the scene to assist. Dump tanks were deployed and a water shuttle was started. The barn and its contents was a total loss with an estimated $200,000 in loss. There was a total of seven departments on the scene with D-4 sending T-45 and T-44. No injuries were reported.
Structure Fire / Raccoon Twsp.
On 1-16-2010 at 21:24, D-4 responded to 1275 Adamsville Rd on a report of a Structure Fire. On arrival we found fire in the wall behind a gas heater insert. Members worked to make access into the wall and the fire was extinguished. Fire damage was contained to the wall of origin with minimal water damage to the interior of the residence. There were no injuries. Units on the scene were T-41,T44, T-45, T-40 and T-47.
MVC Rollover / Entrapment in Perry Twsp.
On 1-7-2010 at 20:11 D-4 responded to the area of Nebo Rd and Wolfe Run Rd on a report of a county dump truck that had flipped over with reports of the driver being trapped. Upon arrival the truck was lying downhill in the road on its drivers side with the driver still in the vehicle. Patient was stabilized by EMS personnel and extricated from the vehicle by the FD. Patient was transported to the hospital by Gallia County EMS. Units on scene were T-47, T-40, T-41, M-94, M-92, OSP and the Gallia County Sheriff Dept.
